Tell us here the exact information you want Emergency Services / Paramedics etc. to access via your Coded ID Labels. Give as much detail as you would want to say if you were able to say it to them in person.

PLEASE NOTE: In order to give this service free for any period, we make a small charge (from 50 pence) for updates to your information so PLEASE be as certain as possible that you include everything first time.

INFORMATION WHAT TO INCLUDE

• Your name
• Your next of kin's name / contact number
• Next of kin's address
• Next of kin's email address
• Any medical condition you have
• Full list of medications
• Any contra-indications (No Penicillin etc)

Consider also including: - 

• Blood group
• NHS number
• Beliefs / faith
• How that affects how you would like to be treated
  
• Diet
• Organ donation

Remember these are optional. The list of things that you can include is endless. So long as the main emergency requirements are included, the rest is personal choice. And there is no right or wrong. 

UPDATING YOUR INFO LATER

You may update your information as frequently as you wish to. During any free service period we make a small charge for updates (from just 50p) which you may pay for on an ad-hoc basis or by a small monthly subscription.
